Below are situation and resource request forms that should be sent to the Medical Health Branch of the County Emergency Operations Center by the medical/health community (i.e.: Hospitals, Clinics, Skilled Nursing Facilities, etc) to communicate with the Medical Health Operational Area Coordinator (MHOAC) in a disaster.
